The Foundation for Care Management is a 501(c) 3 foundation governed by a distinguished board of healthcare professionals.  FCM is accredited to provide continuing education for physicians, nurses, and pharmacists.  Its mission is to provide evidence-based courses for primary care providers who desire continuing education of the highest quality.

This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint sponsorship of the Foundation for Care Management and The Ageless Regenerative Institute.  The Foundation for Care Management is accredited by the ACCME to provide continuing medical education for physicians. 

The Foundation for Care Management designates these educational activities for a maximum of 16AMA/PRA Category 1 Credits™.   Physicians may only claim credit commensurate with the extent of their participation in the activity.
